PCC provides wholistic support to all pregnant women, regardless of income, through care coordination services and community partnerships. To support pregnant women and address critical barriers to care, we provide:
Access to Prenatal Care: Our care coordinators connect you to affordable, timely, and convenient prenatal care.
Appointment Coordination & Scheduling: We help schedule prenatal appointments to ensure timely care through partnerships with hospitals and providers.
Referrals for Additional Assistance: We work with community partners to ensure you can attend critical medical appointments and are connected to other services, including substance abuse treatment and recovery support.
